BD\u2019s Helping Build Healthy Communities Innovations in Care awards grants to community health centers to advance their innovative care models.<\/span><\/p>\n<p><span style=\"font-weight: 400\">This award is intended to help community health centers expand existing programs that focus on the integration of pharmacy-led comprehensive medication management (CMM) of chronic conditions. These programs include Spanish-language CMM services and SDOH screening. SDOH, such as transportation, access to healthy food, social support, and economic stability, can pose barriers to at-risk populations\u2019 access to healthcare.
