Dear Faculty and Instructors,

If you haven’t done so already, now is the time to poll or survey your students about returning to in-person instruction after Spring break. It is important not to exceed the COVID-19 capacity of any room. As of 3/18/2021 we are currently in the RED TIER.

To help save you some time, we have created a simple Canvas survey we can copy to your Canvas course. If you would like to have this survey copied to your Canvas course, please fill out this short form. The survey will be copied to your Quizzes tab and it will be in an unpublished state. You will have full ability to edit the survey to your liking, change the settings, and distribute it to your students at your convenience.

If you do not use Canvas or if you prefer to use a different tool, we also have a survey available in Google Forms.

To retrieve the Google Form, visit the Chapman Google Forms template gallery and copy the form to your Google Drive. Note: You will need to be signed into Google with your Chapman credentials to see the Chapman Google Forms template gallery.
