Jennifer Ko, Pharm.D., has received a grant from BD, Direct Relief, and the National Association of Community Health Centers in the amount of $225,000 for her proposal “Meeting patients where they are: Addressing social determinants of health (SDOH) through Spanish language-concordant and mobile clinical pharmacy services.”

This proposal was submitted on behalf of Family Health Matters Community Health Center, Ko’s practice site. BD’s Helping Build Healthy Communities Innovations in Care awards grants to community health centers to advance their innovative care models.

This award is intended to help community health centers expand existing programs that focus on the integration of pharmacy-led comprehensive medication management (CMM) of chronic conditions. These programs include Spanish-language CMM services and SDOH screening. SDOH, such as transportation, access to healthy food, social support, and economic stability, can pose barriers to at-risk populations’ access to healthcare. By expanding these programs, Ko and her practice site will be able to identify barriers and improve the accessibility of the care they provide.
